Diabetes Care Designation Equates to Healthier Patients

Physicians on Methodist Medical Staff receive Bridges to Excellence Recognition

Helping patients with diabetes avoid complications that may land them in the hospital, twelve independently practicing physicians at Methodist Family Health Centers and Medical Groups have been recognized in 2012 by the Bridges to Excellence (BTE) program for diabetes care. Fifteen physicians were recognized in 2011.

BTE recognizes and rewards clinicians who deliver superior patient care. These programs measure the quality of care delivered in provider practices. BTE places a special emphasis on managing patients with chronic conditions who are most at risk of incurring potentially avoidable complications.

charlton_logo“Patients with chronic conditions whose physician participates in the BTE program generally had fewer hospital admissions and were healthier on average,” says Melissa Gerdes, MD, ABFM, FAAFP, AVP and CMO outpatient services and ACO strategy and Methodist Family Health Centers BTE program clinical director. “Practices with recognized physicians reported a higher level of shared accountability between patients and providers. As a result, these patients are better managed and incur fewer potentially avoidable hospitalizations and emergency department visits.”
